Note: You only need to complete sections for instructors you have worked with. Skip any instructor sections if you haven't had sufficient interaction to provide meaningful feedback.

📚 Purpose

As a divemaster trainee, you have unique insights into our instructors' teaching methods, safety practices, and customer interactions. This feedback helps us maintain the highest standards in diving education and identify areas for improvement.

📝 What You'll Be Asked

For each instructor, you'll evaluate:

Teaching Quality: How well they explain concepts and conduct training
Customer Interactions: Their professionalism and behavior with customers
Crisis Management: How they handle challenging situations
Safety Practices: Your confidence in their safety leadership
RAID Standards: Their compliance with certification standards

🎯 How to Complete This Form

Skip instructors you haven't worked with - Only complete sections for instructors you can meaningfully evaluate
Be honest and specific - Provide examples when possible to help us understand your perspective
Rate consistently - Use the 1-5 scale where 1 = Poor and 5 = Excellent
Include both positives and areas for improvement - Balanced feedback is most valuable
